Travel Vaccinations

Travel vaccinations have resumed at Ashfield Surgery.

To book an appointment with one of our nurses please fill in and complete the Travel Risk Assessment form located on the Travel Page of our website.

In order to complete the risk assessment and any subsequent vaccination schedule safely we must ask that you book your appointment no less than 6-8 weeks prior to departure. If you are travelling in less than 6 weeks then you will need to book an appointment at a private clinic.
