Appointments with the Nursing team are NOT booked via the online form. Please call the practice as normal to book these appointments.

You can call, walk in, or ideally use our online request form and answer some simple questions about your needs. The system will then assess your responses and provide you with appropriate local services, self care, or a clinical appointment where needed.
Can you still book appointments with specific doctors, can I book either a telephone or face-to-face appointment?
Yes you can. When you have completed the online form, you will be presented with a list of suitable appointments. You can filter by specific clinicians and whether the appointment is face-to-face or telephone.
Sharing Information With Our Call-handlers
Irrespective of how you make your request (online, face-to-face or via telephone), all requests for an appointment will go via our triage system. Our call handlers will need to ask you information in order to allow our triage team to make the most appropriate decision regarding your care.If you are uncomfortable with sharing this information with our call-handlers, please use the online appointment request option to complete the information yourself.
We will not be able to make exceptions for patients unwilling to share this information with our call-handlers or submit the information themselves online.

Extended Hours HUB will be at Ashfurlong Medical Centre Ashfurlong Medical Centre, 233 Tamworth Road, Sutton Coldfield, B75 6DX.
Weekday evening appointments are telephone only. Face to face appointments are available at the hub on Saturday.

Livi is working in partnership with Ashfield Surgery to provide FREE GP video consultations for our patients; appointments are booked via the Livi app with is available on most smartphones via the app store.

They offer a range of appointment times to suit your needs including evening and weekend appointments and can cover a range of medical needs including coughs and colds, fever, fit notes, UTIs, anxiety and depression, and many other health queries.

Cancellations
Once you have been given an appointment time, please ensure you are available for the GP or nurse for telephone consultations. Demand on clinician time is very high , and not being available or non-attendance of booked appointments is a waste of valuable time which increases the general waiting time for an appointment. Patients are expected to give advanced notice (at least 1 hour) if they are unable to attend. Repeated failure to do this may lead to removal from our list.
