CORONAVIRUS VACCINATION NEWS ***IMPORTANT***

The Coronavirus vaccination is now available to our patients aged 80 and over.  Please do not phone the surgery, we will call all the patients in this group and advise where and when the vaccination can be given.  These vaccines are administered for several surgeries within the Primary Care Network, and COVID vaccine is not supplied directly to us at Ashfield Surgery.

PLEASE NOTE:  ALL GP APPOINTMENTS REMAIN TELEPHONE CONSULTATIONS ONLY, THE GP OR NURSE WILL CALL YOU AS CLOSE TO THE APPMT TIME AS POSSIBLE.  IF THE GP FEELS YOU NEED TO BE SEEN FACE TO FACE, THEY WILL MAKE ARRANGEMENTS FOR AN APPOINTMENT AT OUR AMBER CLINIC WITH YOU DURING THE CONSULTATION,  WEARING FULL PPE.

OUR NURSING TEAM ARE SEEING PATIENTS IN FULL PPE FOR INJECTIONS,  WOUNDCARE, BLOOD TESTS CHILDHOOD IMMS & SMEARS.  PLEASE CALL THE APPMTS LINE. REVIEW OF DIABETES AND RESPIRATORY LONG TERM CONDITIONS REMAIN TELEPHONE CONSULTATIONS.

4.  Care of your ears: Ear Syringing is used to clear ear wax from ears that is causing reduced hearing. This service will no longer be available at the surgery. We have put together an INFORMATION LEAFLET designed to help you care for your ears.

register_nowNew Patient Registration

The practice welcomes and serves any patients within the B76 postcode area and eligibility can be quickly confirmed from your address. In this respect it would help if you could provide proof by way of a recent utility bill.

You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. You will be allocated a named GP, but if you have a preference please inform Reception staff at the time of registration.

All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.   Medical treatment is available from the date of registration. Please contact reception for further information.

Registration With The Practice

You will need to come into the Practice and complete registration forms and a medical questionnaire.  Please bring your NHS card with you if possible.  If you are unsure whether you live within our Practice boundary (B76), please speak to one of the Reception Managers who will check for you.

We are unable to offer On-line Registration due to the restrictions on our practice boundary, which causes confusion for patients wishing to register with us but who live outside the B76 practice area.

Temporary Registration

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
